The following Site Preparation Checklist form (two pages) requests specific information about your installation site. To ensure a successful installation by reducing last-minute complications, verify the preinstallation requirements discussed earlier in this guide, and complete and return the Site Preparation Checklist form to Cisco Systems.
The Site Preparation Checklist form should be filled out by the installation site contact person. After the form is completed, it must be faxed to Cisco Systems.

Select one of the following options for loading a software configuration on your system:
